Please be advised that, effective April 24, 2009, by Commissioner's Order issued by the Commissioner of the New York State Department of Health, Ram Swaroop Makker, M.D., was ordered not to practice medicine in New York State or in any other jurisdiction where the practice of medicine is predicated on a valid New York State medical license. Effective May 6, 2009, Dr. Makker voluntarily resigned his authorization to treat workers' compensation claimants. No current business address is available for Dr. Makker.

Dr. Makker is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Makker for treatment rendered prior to April 24, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Makker regarding treatment he rendered prior to April 24, 2009, should not be denied due to the Commissioner of Health's Order or the resignation of his authorization.
